Required for all Competitive programs. Should be reviewed and updated at registration or when conditions change. Optional for all other programs unless there is a medical condition that the coach / coordinator should know about that could impact the health or safety of the child/participant. In this case, providing medical information is at the discretion of the parent or guardian.

Personal Information Protection & Electronic Documents Act (PIP&EDA) consent is required one time only when members first register with a water polo club in the province of Saskatchewan. The consent is considered active until a request is made to withdraw consent. The PIP&EDA form for Water Polo Saskatchewan (WPS) must be completed for each participant (coach, official, athlete, volunteer) that is registered in the on-line database. Refer to the WPS PIP&EDA policy and consent forms as shown on the Water Polo Sask Website.

Release and Compliance agreement is required for travel and must be discussed and handed in prior to travel. RWPA follows the Water Polo Canada guidelines, using the respective WPS-COC for travel within Saskatchewan or the WPC-COC for travel outside of Saskatchewan.
